Bridging the gap between schools and opportunities.

Transitions from primary to secondary school can be challenging, especially for vulnerable adolescents. SmoothTransition supports young people, teachers, and families by providing tools and methods that make this change smoother. The project strengthens inclusion, builds confidence, and helps schools create environments where every student can thrive.

School transitions are changing. We’re supporting the change.

Our partnership brings together educators, youth workers, and specialists to create tools that help adolescents adapt to secondary school. By focusing on inclusion, confidence, and engagement, we’re shaping a smoother path for young people and their communities.
